Do you give your consent for Winnipeg harvest to photograph and/or film for the purpose of raising awareness of poverty issues, food collection, time and money? This photograph may be reproduced, used and distributed whether alone, or in connection with other photographs in any and all manner and media perpetuity. The permitted uses include, but are not limited to, all forms of print, electronic and multi-media reproduction including software programs on the internet.

While we try to place every prospective volunteer, we reserve the right to reject applicants who do not meet our requirements and/or criteria.
Release from Liability:
Volunteers release Winnipeg Harvest and its agents, representatives, trustees, officers, employees and volunteers from any liability arising out of any damage, loss or injury to the volunteer or his/her property incurred as the result of volunteer activities.

1. My child will work in an environment where forklifts, pallet jacks, and trucks are in use and where heavy boxes of food are stacked on pallets.
2. My child will work in an environment that may be hot, cold or dusty.
3. My child's work will not be supervised one-on-one.

Furthermore, I understand and agree that submitting this application form does not automatically register me as a Team Member (volunteer or a training program participant). It is the policy of Winnipeg Harvest Inc. to screen all prospective team member. I understand that I will be advised in advance if a criminal record check and/or a driver's abstract or other program specific checks may be required. While we try to place every prospective team member; we do our best to placements based on your skills, interests and availability with Winnipeg Harvests requirements and needs at the time of application.
NOTE: Volunteers must wear close-toed shoes. Volunteers wearing open-toed shoes will not be allowed in the warehouse and will be asked to leave.
